Ukrainian Tourist Visa

A valid original passport with at least 6 months validity and at least 1 blank page available for the visa.
A completed visa application form (the form must be filled out completely. No blank spaces must be left. If a question does not apply, please type N/A.
One passport size photographs
A copy of the tourist voucher and copy of confirmation invitation ( through our Company )a Belarus travel agency which shows the refrence number for the visa.
Onward/return tickets or confirmed itinerary
